Public opioid surveys

The AZSCIA is dedicated to building a healthy

community for individuals with spinal cord injuries

and their families. We are conducting data collection to

understand the use of opioids within the disability

community. The goal is to understand the role opioids

play in response to chronic pain. Your feedback is valuable

in creating new opportunities to educate and identify

resources to increase awareness around the opioid epidemic

in Arizona. Your responses are confidential and will not be shared with any third-parties. We appreciate your time and appreciate your participation!

On a final note, we are honored to begin this groundbreaking work. The State of Arizona is dedicated to saving the lives of those affected by OUD. Our work will provide a foundation for future policy decisions that will directly impact how OUD and opioid use is handled within the SCI community and overall disability population. One of the most important pieces of data we can collect is real personal stories about how OUD has affected you personally or a loved one. If you have a story to share, please give us a call. All stories will remain confidential. We want to understand the experience you have had with pain management opioids, whether you have accessed treatments, and overall quality of life viewpoints that can provide context behind the work we are doing. Call our office and set up a phone call or in-person appointment to share your story.

Below, we have put together surveys which can be used to capture important data that we will utilize to discover the pulse of the epidemic in our community. Once again, these responses are confidential and your information will not be shared with any third parties. We have a separate survey for individuals and another one for family members and caregivers. These are unique stories that can give the AZSCIA a better understanding of the needs and gaps in service that exist.
