Fusion All-Stars is dedicated to having one of the best programs in Utah! To make this possible, we have established rules and policies that
we feel can make us the best. This sheet will help to inform the cheerleader, as well as the parent/guardian of their responsibilities as a
member of this program. The rules and policies are subject to change without notification.
I understand by joining Fusion All-Stars the following rules and policies must be followed:
• I understand that remaining current with tuition, other Fusion All-Star charges and financial obligations are my responsibility. My
child will not be able to participate if tuition is more than 30 days late. Once paid, my child may resume with the team. If this becomes a
