Director of Insurance, Hyderabad letter No. 1/General/ 2012-2013, dated 21-03-2013. * * * O R D E R: In the Governnment order 1st read above, the Governnment have constituted a Committee too study the functioning and procedures followed in the A.P.G.L.I. Department and to makee recommendation for simmplification of the same to submit Propossal Forms. 2. The Committeee in the reference 2nd read above, has su bmitted a report to Government makingg certain recommendationns for effective functioning of the deparrtment, recommending not to insist thee Proposal Form for additiional policies, since the employees have already furnished certain information inn the Proposal Forms submmitted towards fresh insurance. 3. The Director oof Insurance, Hyderabad in his letter 3rd ccited has submitted the proposals on thee recommendations of thee Committee and stated that he may be permitted not to insist for the proposall forms for issue of addittional policies to the extent of compulssory slab and the Drawing & Disbursingg Officers may be instructted to submit the information with regaard to the recovery of further premiumm details, such as Name off the Employees, Designation, Father’s N ame, Date of Birth, Policy Number, Basicc Pay, amount of premiumm increased and month and year in whhich the increase is effected, so that thee Director of Insurance maay prepare the additional insurance poli cies and issue to the concerned throughh respective Drawing & Diisbursing Officers,. In respect of fresh innsurance and in case where premium iss deducted more than commpulsory slab, the submission of proposaal forms are compulsory. 4. After careful e xamination of the proposal of the Dirrector of Insurance and accepting thee recommendation of the committee, Government hereby
order tthat there is no necessity for submissionn of proposal forms for addditional policies here after to the extentt of compulsory slab. In respect of freshh insurance and in the ccases where the premium is more thaan compulsory slab, the submission off proposal form to the resspective District Insurance Office is comppulsory for issue of policies. 5. All the Drawing aand Disbursing Officers are therefore reqquested to furnish the information to thee concerned District Insurrance Officers showing the details such a, Name of the Employee, Designation,, Father’s Name, Date of Birth, Policy Number, Basic Pay, amounnt of premium increased and month andd year in which the incrrease is effected, after recovery of incrreased premium from the salary of thee employees without fail. They are also solely responsible for obttaining additional policy to the extent off premium recovered and to mentioned the correct Policy Numbbers in the monthly schedules. 6.
