A Closer Look: My Chart Communication is a key element of the Patient Experience. Three of the HCAHPS measures are communication with doctors, communication with nurses and communication about medications. Communication engages our patients in their care allowing them to have a positive patient experience. Recently, Providence Health Center took steps towards increasing our patient’s engagement by providing them with an avenue to access their health summary through an application called MyChart. MyChart is an online application that connects patients with a portion of their medical record, providing access to information regarding their hospital visit. MyChart enables patients to securely view their medical information while in the hospital, from the comfort of their own home, or with their mobile devices through the MyChart app. Providing this tool increases engagement by helping patients understand their plan of care as well as by providing the information patients need to make better informed healthcare decisions. There are many benefits to this new application: MyChart acts as reinforcement to the information that patients receive from their care teams while they are in our hospital. Patients who are educated on their medical condition are more apt to comply with prescribed medications thus improving outcomes, decreasing readmissions, and, for those with chronic conditions, have a greater positive impact on their health, safety and overall experience. MyChart also enables patients to share needed information with other providers, potentially avoiding unnecessary procedures or duplicate tests. If there is ever a need for a patient to recall what test or medication was given during their time in the hospital, MyChart can link the patient with that important information. A team of dedicated Providence volunteers has been trained to visit patients in their room to register them on the MyChart app. By registering patients during their stay in the hospital we engage them to participate in their medical care creating a culture of safe, excellent, person-centered care.
