Steve Carstensen, DDS has been treating sleep apnea and snoring for 18 years, has
completed training at UCLA’s Mini-Residency in Sleep, and is a Diplomate of the
American Board of Dental Sleep Medicine. He lectures internationally, directs
sleep education at the Pankey and Spear Education, and is a guest lecturer at University of the Pacific
and Louisiana State Dental Schools. For the AADSM he was a Board Member, Secretary-Tresurer, and
President-Elect. From 2007-2010 Steve served the ADA on the Council for Annual Sessions and was Chair
of the 2010 ADA meeting. Together, with a partner, he founded Premier Sleep, a practice in Bellevue, WA
entirely devoted to helping people breathe better and sleep better.

About; Medical Encounter Notes
Dental notes focus on what we’ve done for our patients at each visit. Medical notes must record what our
patients present, what we’ve found, what we’ve thought about, and what we recommend. Then, we must send
these notes to the patients’ other healthcare providers.

Steven D. Bender, DDS earned his Doctorate of Dental Surgery degree from Baylor
College of Dentistry in 1986 and practiced general restorative dentistry for 14 years.
He then studied orofacial pain and temporomandibular disorders at the Parker E
Mahan Facial Pain Center at the University Of Florida College Of Dentistry. From
2000-2015, Dr. Bender maintained a private practice devoted to pain management
of the head and face, as well as sleep medicine. Beginning in 2016, he transitioned to a full time faculty
member of Texas A&M College of Dentistry and assumed the role of director of facial pain and sleep
medicine. He has earned Fellowship in the American Academy of Orofacial Pain, the American Headache
Society, the International Academy of Oral Oncology and the American College of Dentists. In addition,
he serves as a consultant for the United States Army.

As the role of dentists in sleep medicine continues to evolve, education, research, and ultimately standard-
ization of the field is critical to optimum patient care. Bringing unique insights gleaned from both clinical
experience and academia, Dr. Bender will provide his vision of the future of dental sleep medicine.
