WAIVER AND RELEASE OF ANY AND ALL LIABILITY

PRIOR TO PARTICIPATION IN ACTIVITIES

On behalf of the above named Player, I hereby acknowledge that I have voluntarily applied on behalf of the above
named Player to participate in practices and soccer games affiliated with West End Soccer Organization for the 2013
season.
I AM AWARE THAT SOCCER IS A POTENTIALLY HAZARDOUS ACTIVITY, AND CONSENT TO THE PLAYER’S
PARTICIPATING IN THIS ACTIVITY WITH KNOWLEDGE OF THE DANGER INVOLVED AND HEREBY AGREE TO ACCEPT ANY AND ALL RISKS OF INJURY OR DEATH.
I HEREBY RELEASE AND DISCHARGE WEST END SOCCER ORGANIZATION, TOGETHER WITH ITS SUCCESSORS, ASSIGNS, TRUSTEES, OFFICERS, EMPLOYEES, REPRESENTATIVES AND AGENTS, OF AND FROM ANY AND
ALL CLAIMS OR LIABILITY OF ANY TYPE WHATSOEVER, INCLUDING, BUT NOT LIMITED TO, PROPERTY DAMAGE,
PHYSICAL INJURY, MENTAL ANGUISH, EMBARRASSMENT, DEFAMATION, AND INVASION OF PRIVACY, WHICH
THE PLAYER MAY SUFFER ARISING OUT OF, BASED UPON, RESULTING FROM, OR IN ANY WAY CONNECTED WITH
THE PLAYER’S PARTICIPATION IN WEST END SOCCER ORGANIZATION ACTIVITIES, INCLUDING BUT NOT LIMITED TO ANY CLAIM ARISING OUT OF, BASED UPON, RESULTING FR OM, OR IN ANY WAY CONNECTED WITH THE
NEGLIGENCE, OMISSIONS, OR OTHER ACTS OF WEST END SOCCER ORGANIZATION, ITS SUCCESSORS, ASSIGNS,
TRUSTEES, OFFICERS, EMPLOYEES, REPRESENTATIVES OR AGENTS, OR THE CONDITION OF ALL OR ANY PART OF
PREMISES USED BY WEST END SOCCER ORGANIZATION.
I acknowledge and agree that I and the Player understand that the foregoing release means that if the player is
injured, or any property of a player or any part thereof is damaged while the player is participating in a West End Soccer
Organization activity:
A.
The Player will not be able to recover from West End Soccer Organization, or any of its officers, directors,
employees, representatives or agents for any of the above-described injuries, losses or damage suffered thereby, even if
such injuries or damage are due to the acts or omissions of West End Soccer Organization, or its successors, assigns, trustees, directors, officers, employees, representatives or agents.
B.
The Player will not be able to make a claim for or file a lawsuit against West End Soccer Organization, or
successors, assigns, trustees, directors, officers, employees, representatives or agents even though West End Soccer Organization, its successors, assigns, trustees, directors, officers, employees, representatives or agents may have been liable to
the Player.
C.
I/ We hereby give consent to have my/our child photographed in the daily routine while at West End Soccer. Such photos are the property of West End Soccer and the photographer and may be used in sponsorship and promotional material relating to the West End Soccer League.
I have carefully read this Agreement, and fully understand its contents. I am aware that it is a release of liability,
and a valid contract for consideration between me, and on behalf of the Player named herein, and West End Soccer Organization, and I have signed it of my own free will.
