What are we aiming to achieve?
We recognise that carers and
family members may need
different things at different times,
and that ensuring their needs are
met is a collective responsibility.
By working collaboratively with
our local community and other
services we will develop our
current approach to supporting
carers and family members
to ensure that locally anyone
caring for, or supporting
someone living with an incurable
illness, can be supported and
enabled to fulfil their vital role in
caring, for as long as they wish,
whilst maintaining their own
health and wellbeing.
How will progress be
monitored and reported?
Through consultation and
engagement, both internally
and externally we will develop
and launch a strategy for Family
Support and Carers. This will be a
three year strategy and a detailed
operational and implementation
plan will be developed alongside
the business plan. This will
necessarily include financial
modelling and plans for
identifying potential funding.

4.3 Ensuring interoperability
of electronic patient record
systems
How was this identified as
a priority?
Our current electronic system
has been in place for over 10
years and has some limitations,
the most important of which
is that it cannot ‘talk’ or share
information with other systems
in use by our local NHS partners,
such as GPs.
The ability to view or share
records between other health
care teams and ourselves in real
time would improve patient care
by ensuring clinical information is
up to date, readily available and
that care and management plans
are shared and understood by all
healthcare professionals involved.
This in turn ensure decisions are
made in a timely way and in the
best interest of patients.
What are we aiming to achieve?
We are working with
commissioners, neighbouring
hospices and our local health
economy partners in care to
explore this together. There is
work happening across our local
NHS concerning how healthcare
information can best be shared
to support improved patient care.
This will help us decide which
healthcare record system we
should switch to. Our Trustees
will support investment to make
this happen once the right
system has been identified.
How will progress be
monitored and reported?
This is a strategic priority for our
Board so our progress will be
reported on and discussed at
every Board meeting.
How will we know what we
have achieved?
Ultimately we will have identified
the right system to support
interoperability with other
healthcare record systems and
have a plan for implementation.
These plans will include
milestones and outcomes
which will be reported to our
Board quarterly.

Although the strategy itself will
cover three years, it will provide
a clear vision and plan which will
be shared across our catchment
with public and professionals
and all those involved in its
production. It will clearly set out
what families and carers can
