3. Looking Back: Priorities for improvement from 2018 – 2019 3.1 During 2018/19 we widened access to inpatient care for our local community What we did? We invested in a reconfiguration of part of our inpatient unit in order that we could offer six new single en-suite rooms for people who are at end of life and eligible for fast-track Continuing Healthcare Funding (CHC). This included social spaces for both residents and families as well as access to daily activities. These beds enabled us to increase our overall provision of inpatient care and widen access to people who needed a different type of care. Previously this type of care would have been provided in a care home as the person’s care needs would not require the input of our full specialist inpatient team. 6 What was the outcome? Following the reconfiguration in January 2018 all six beds were opened and were operational by April 2018. Since that time, up until end of March 2019, we have cared for 27 people. Three of these people remain resident at the time of reporting. The majority of people remained with us for less than a month with three staying with us for over three months. In some cases, people have been able to transfer to our CHC beds after having their care stabilised by the specialist team, rather than face a further move to a care home which supports their continuity of care in familiar surroundings. “Tony experienced the ‘end of life’ care that he wished for.” “Amy, Trudi and the rest of the team were so helpful and professional.” “St Giles is a wonderful organisation.” “Thank you for your dedication, kindness and empathy to our loved one while he was in your care.
