friends, recognising the vital

evelop a rapid response service
for people in our community
who urgently need support in
managing their symptoms eek further opportunities to

of people dying in hospital when
that is not their preferred place
of care

new ways of managing often
complex symptoms such as pain,
breathlessness, poor nutrition
and fatigue in order to improve
people’s ability to live a fulfilling
life for as long as possible
