Ty in a second clump of buck tail (a littler clump than the first). Allow the hair to spin around the hook evenly.

Trim the tag ends so they do NOT extend forward beyond the most forward warp of lead wire.

Secure with several wraps of thread.

Create the head:

Cut the round foam into a 5/8 inch length

use a pick or large needle to puch a hole through the center.

Trim one end at a 45 degree angle.

Test fit the head onto the hook

If the fit is good remove and apply a layer of glue (I use Gorilla glue) to the hook shank.

Push and align the head quickly, you don't have much time before the glue sets.
