Depending on your child’s vaccination history, certain vaccines may be needed.

vAccinAtions for your chilD:
Ages 4-6
Ages 16 and older
DTap
• Polio
• MMR--2nd Dose
• Varicella--2nd Dose
• Meningococcal booster
• Varicella--If your child has not had the
first dose by age 13, two doses are
needed.
• HepB--Recommended if your child has
not had the shots. Three doses are
needed.
• MMR--If your child has not had the first
dose, two doses are needed.

Ages 11-12
• Tdap--Required
• HPV--Recommended for boys and
girls to prevent most genital warts and
cervical cancers as well as some other
cancers. This is a 3-dose series.
• Meningococcal--1st dose
• Varicella--If your child has not had the
first dose by age 13, two doses are
needed.
Flu vaccines are recommended
for all children over six months
of age and will be available
beginning in the Fall.
