Loss Prevention Bulletin (LPB) Our Loss Prevention Bulletin is the leading source of case studies in process safety in the industry and has an archive spanning over 40 years of lessons learnt from accidents. It was first published in response to the disastrous explosion at Flixborough in 1974 when a need was established for the process industries to be able to share information on accidents in the hope that a lesson shared is a lesson learnt.
