Daily Care Hourly Rounding A caregiver will check on you every hour during the day (6 am to 10 pm) and every two hours at night (10 pm to 6 am). During this time we will be checking on your well-being, monitoring comfort and pain, helping you move and change positions and assisting with trips to the bathroom. Bedside Handoff of Care Bedside handoff of care occurs at shift change when the nurses coming on and going off duty meet by your bedside to talk about your care. This gives you a chance to meet the nurse taking over your care, ask questions and share information. Whiteboard Your room has a whiteboard which lists who your caregivers are for the day and any special information you and/or your caregiver need to know. If you have questions for your doctor, have a caregiver write a note on your whiteboard as a reminder to the care team when they round. Quiet Time Rest is an important part of recovery. From 9 pm to 7 am, we keep it as quiet as possible for you to sleep. During this time, hall lights are dimmed, overhead pages are limited and phone calls are not transferred into patient rooms (emergency calls are sent to the nursing station). Activity Physical activity is important to the healing process and helps to prevent pneumonia, blood clots and skin problems. Your doctor will write an activity order as part of your care plan which may include working with a physical or occupational therapist. “Find” Code If you would like family members to receive detailed updates on your care, we have found that it works best if you choose one or two people to be the contacts that talk with the nurses and can then update others. In order for us to share information, you must give the “find” code you received on admission to your contacts as we will only release information to those who provide us with your “find” code. 3
