Care Team Care Team Your care team consists of trained professionals who put your safety and well-being at the center of all they do. We work as a team and each person caring for you has special knowledge and skills. Your care team will vary based on your personal needs but may include:  Physician — determines a care plan based on your needs and desires. You can expect to see your doctor at least once a day. Sometimes a hospitalist will care for you instead of your family doctor. A hospitalist is specially trained in hospital medicine and will keep your family doctor informed of your care.  Nurse — provides your care and monitoring, ensures your daily needs are met and provides education and information so you can understand your condition and treatment.  Patient Care Assistant (PCA) or Tech — takes vital signs, monitors glucose levels and helps you bathe and get ready for the day. Techs may draw blood and do EKGs.  Respiratory Therapist — provides breathing treatments and oxygen.  Physical Therapist — helps with walking and exercises to build strength.  RN Case Manager — helps coordinate your hospital stay and plans for your needs after discharge.  Pharmacist — helps ensure you receive the right medication at the right dose.
