To be eligible for
kindergarten, your child
must turn
5
on or before
August 31, 2018.
To Register, You Will Need to Bring:
• Certified Copy of Your Child’s Birth Certificate or
other Evidence of Child’s Date of Birth
• Your Child’s Updated Immunization Record
• A Completed Kindergarten Health Assessment Form
(Forms are available at all elementary schools and Health
Departments.)
• Proof of Residency (i.e.: current utility bill, property deed, or
rental lease agreement)
