Your File

One of the first things PANIC advocates advise you to do is to get a copy

of your CHILD, YOUTH & FAMILY SERVICES file.

A form for this can be posted directly to you from PANIC, or can be

printed, filled in and posted by you HERE . We will also send out our

brochure and other brochures from the ministry that contain relevant

information. This pack is sent to you free of charge and also includes an

extra O.I.A form that can be used for medical or police files. If you would

like an information pack please use CONTACT US. Put in the ‘comments’

section: information pack and give us your details. One will be posted to

you as soon as possible.

Why is it important you have your file?

Your file contains all the notes social workers have uploaded into the CYFS computer. These can date back many years or just a few weeks. It will have notes on allegations or concerns made about you and your children. It may also contain other information unknown to you.
