Surgical Guide The use of lasers with a “closed approach” was contraindicated in this case due to the irregular osseous anatomy and presence of buccal bony ledges. Due to the extensive zone of keratinized gingiva, it was possible to utilize submarginal incisions without causing a mucogingival deformity. Full thickness buccal and palatal flaps were raised to expose and recontour the osseous architecture accordingly in order to provide a harmonious anatomy and adequate space for the biologic width. 5-0 Vicryl vertical sutures utilized for closure. Occlusal analysis and adjustment with delivery of a maxillary occlusal nightguard was utilized to protect the dentition and address parafunctional habits. The interdisciplinary periodontal/restorative approach was able to meet the patient’s expectations and obtain a result that is compatible with health, comfort, function and esthetics.
