Dear Colleagues :
It is with great pleasure that all of us at Mount Carmel Health Partners present you with our annual report for 2016 . We ’ ve been fortunate to enjoy extraordinary growth and success over the last year , particularly in the areas of clinical integration , accountable care , and quality improvement , and are pleased to share some of the highlights with you .
We spent the last several years preparing our organization for the challenges of the ever-changing landscape of healthcare reform , and all the evidence suggests that our strategies of preparedness and foresight were on target . In the past year , we exceeded our goals for provider engagement in our clinically integrated network . We expanded our accountable care partnerships to nine , and our population health management efforts were greatly enhanced by increasing both our programming and staff .
As a result of this remarkable growth , Health Partners was able to make significant investments in the future in the areas of infrastructure and data analytics . We also increased our commitment to patients by supporting our member physicians in delivering the highest-quality healthcare as effectively and efficiently as possible .
We owe a great deal of our success to the ongoing support of our member providers — primary care and specialty physicians , advanced practice providers and hospital partners . We couldn ’ t do what we do without each one of them . Our future is bright as we continue to collaborate across the continuum of care to provide our patients with the highest-quality healthcare .
