MAXIMIZING INVESTMENTS IN DATA ANALYTICS AND INFRASTRUCTURE
In light of Mount Carmel Health Partners ’ remarkable growth , investments in infrastructure and data analytics are key to our sustained success . To that end , we ’ ve made a number of investments in technology to support our members and to improve the quality and efficiency of care and promote an evidence-based , outcomes-driven approach to care .
» CareEvolution . An internal intelligence tool that allows us to share clinical information for attribution , population utilization , risk score , high-risk patients , gaps-in-care reporting and trending in a secure , reliable , and incremental manner .
» Verisk Health Enterprise Analytics . An internal tool used to assist in the management of program performance , population health , enterprise reporting , budgeting , network efficiency , risk contracting and ACO initiatives .
» Quality Connect . An integrated web portal that gives our physicians and their practice staff members tools and reporting capabilities to better and more easily manage their patients ’ care , reach key quality metrics and maximize incentive payments .
» Crimson Medical Referrals . A web-based workflow platform for physicians that facilitates the referral process and keeps care within the network ( out-of-network referrals have decreased for our employed medical group practices since its implementation ).
» CACTUS Credentialing Database . An internal administrative tool that stores credentialing and recredentialing data , automates some of the credentialing processes , and helps ensure that member providers are current with board certifications and licensures .
» The McKesson Population Manager ( formerly MedVentive ). An electronic tool that collects , analyzes and aggregates claims data from physicians , hospitals , laboratories , pharmacies , and other healthcare providers in order to manage populations , track individual patients , and provide drill-down access to data on clinical events and lab results .
» Mount Carmel Provider App . A secure , HIPAA-compliant mobile app ( developed by a Health Partners member physician ) that allows users to send secure messages , including images , to other providers ; makes referrals easy by allowing users to search for providers by name , specialty , group , or campus location ; and gives users easy access to the Health Partners ’ clinical guidelines .
» Data Driven Delivery Systems ( DDDS ). A program that assists providers in capturing chronic conditions , documenting with more specificity , and closing gaps identified by the Stars program to better enable physician collaboration while driving cost savings .
