Senior Services : ( 65 and over )

• MNChoices Assessment : Services to help seniors make decisions about long-term care with the goal of people remaining in the community ; includes early intervention visits , pre-admission screenings prior to moving into a nursing home , information about public and private programs that can pay for services , transition assistance to help people move out of nursing homes and access to community-based services .

• Care Coordination / Case Management - Provided to certain public program recipients ; assistance in locating and gaining access to medical , social , educational , or other needed services ; coordinating plan of care , monitoring services to maximize health and safety , and eligibility reassessments . Family Health Services :

• Family Home Visiting - Prenatal and parenting support , nurse answers questions and provide information and community resources for parents and children .

• Breastfeeding Support - Support for prenatal and postnatal women

• Infant Follow Along - Free developmental monitoring and screening questionnaires for children birth to 3 years old sent to every child born in Morrison County , follow-up by nurse where concerns are identified .

• Child Passenger Safety - Education and training on correct car seat installation and fit , checks car seats for defects and recalls , provides new car seats for specified health programs .

• WIC - Supplemental food and nutrition program for pregnant , postpartum , breastfeeding women and infants and children up to age 5 years old .

Health Promotion :

• Environmental Health - Licenses and inspects food , beverage and lodging establishments , pools , youth camps , recreational camps and mobile home parks ; inspections are made for compliance with state and local requirements ; special event licensing ; tobacco licensing ; public health nuisance follow-up ; resource for air , water quality and home environmental concerns .

• Child and Teen Checkup ( CTC ) - preventative health screening outreach for children birth to 20 years old enrolled in medical assistance .

• Disease Prevention and Control - Responding to and educating the public on emerging infectious disease threats .

• Statewide Health Improvement Program ( SHIP )
