Diabetes Self-Management Education can significantly reduce
A1C, the risk of diabetes-related complications and hospital
admissions and re-admissions.
The American Diabetes Association has identified four critical times
when physicians should refer their patients to Diabetes Self-Management
Education:
• At diagnosis
• On a yearly basis for health maintenance and prevention of complications
• When new complications that influence self-management occur
• During transitions of care
The Louisville Metro Department of Public Health and Wellness holds American Association of Diabetes Educators
Accredited classes every month using an evidence-based curriculum. Classes are open to all and free of charge.
