The All-Hazards Laboratory Preparedness Survey is an annual survey that assesses the challenges and successes of public health preparedness and the capacity of public health laboratories �PHLs� to respond to biological , chemical , radiological and other threats . The survey was distributed to the PHLs of all 50 states , Puerto �ico , the District of Columbia , Los Angeles and �ew York City and yielded a ��� response rate �52�54� .
