WEST JEFFERSON HILLS
Feel better. Be in control. Do the things you want to do.
The Allegheny County Better Choices, Better Health® Program (also
known as the Chronic Disease Self-Management Program) is a free six-
week workshop supported by the United Way of Southwestern Pa and
the Jefferson Regional Foundation and provided in the county through
a partnership with Vintage, Inc. of East Liberty and CCAC. The program
provides tools for living a healthy life with chronic health conditions,
including arthritis, high blood pressure, heart disease, diabetes, COPD,
and other conditions that affect your daily life.
The Chronic Disease Self-Management Program was developed at
Stanford University in 1996. This is a workshop for adults that is held once
a week for six weeks for two and half hours. The workshop is scheduled to
be held this fall at:
Jefferson Hills Public Library, 925 Old Clairton Road, Clairton Pa 15025
Mondays: September 17-October 22, 2018, 1:00-3:30pm
The workshop sessions are led by two trained and certified Peer
Leaders, many of whom also have chronic health conditions. This creates
an environment of mutual support, which builds participants’ confidence
in their ability to manage their health and lead active lives
Positive Outcomes
In a Stanford study, people who participated in the program
demonstrated significant improvements in exercise, cognitive symptom
management, communication with physicians, self-reported general
health, health distress, fatigue, disability, and social/role activities
limitations. They also spent fewer days in the hospital and there was also
a trend toward fewer outpatient visits and hospitalizations.
