the SPS act as a liaison for patient assistance
through our institution’s Medication Assistance
Program (MAP). The SPS and MAP collaborate to
find a solution for a patient’s funding problem.
Once medication access is secured and the patient
initiates treatment, the SPS team provides ongoing
proactive refill coordination via home delivery, pharmacy pick up, or in-clinic drop-off. The SPS also
completes prior authorization renewals at indicated
intervals and provides continued insurance benefits
assessment. They coordinate directly with the hepatology service for any necessary interventions, and
document in the electronic medical record for seamless communication among the team members and
clinic providers. Additionally, the SPS team provides
24/7 on-call services for urgent needs
Conclusion
Patients often struggle with understanding and
adhering to HCV treatment; pharmacists play several
important roles throughout the course of HCV
treatment to help patients complete treatment. They
educate patients from the moment of treatment
inception, ensure access to medications, coordinate
financial assistance, monitor for adherence, and
advise on treatment outcomes to improve the
outcome of HCV treatment and lessen the burden
on the patient. Through the combination of clinical
pharmacist and integrated specialty pharmacy
services, continuous patient care is provided starting
at the time the decision is made to initiate HCV
therapy and continues throughout the course of
therapy.
