Free Chronic Disease
Self-Management Program
Chronic Disease Self-Management
Program is an effective and free workshop
for people with chronic health problems
and care-takers. The program teaches
skills for managing a variety of chronic
diseases. Session topics include
techniques to exercise, use of medication,
communication with family and healthcare
professionals, nutrition, and stress
management. The program meets once
a week for two and a half hours over six
weeks at Baptist Health Lexington Green

class in-progress. Goals: Mobilize your
inner resources for promoting health and
self-care; Learn effective skills for managing the stress of caregiving, burnout and chronic
disease; Study and
