REDUCTION IN HEALTH DISPARITIES/INCREASED HEALTH EQUITY
Goal: Improve Health Equity and Reduce Chronic Disease in Target Populations in Lake
County
T
he Health Department launched a new Health Equity initiative in June of 2015. It is focused on raising
awareness of what health inequities are across the county, and coordinating efforts with various community partners who want to help improve the health of their communities. Based upon proven, national
strategies to improve the health of whole populations, the Health Equity team's approach is designed to
move away from being the proprietor and instead, make health a shared value. The goal for health equity is,
"Everyone in Lake County, no matter who you are, where you live, how much money you make, or your family heritage, has the opportunity to live in a healthy environment and to lead a healthful, fulfilling, and productive life."
The Health Equity initiative is raising awareness of what health
inequities are across the county.
