Successful Transitions for vulnerable young people A NEW PROGRAM is successfully connecting disengaged young people with education and employment. The Successful Transitions program provides intensive case management and mentoring for 17 to 24 year olds. Since it began in 2015, the program has supported more than 150 young people across Northern and Southern Adelaide, the Mid North, Limestone Coast, Adelaide Hills and Murray Mallee regions. Issues addressed include mental health, drug and substance use, low literacy and numeracy, and homelessness. This year, the program will be expanded to Whyalla. Socially isolated, diagnosed with ADHD, autism, and Foetal Alcohol Syndrome, Jack had no school qualifications or work experience. He wanted to find a job and make friends. His Successful Transitions mentor helped him develop a plan and a social and health support network. He joined the Creative Disabilities Program, a social group that builds confidence through acting and singing. Today, Jack has found employment, has friends and improved confidence, and is looking forward to his future.
