inclusive Care for the Elderly
(PACE) Generic term for day
health centers that include a
medical clinic, adult day care,
social services, rehabilitation,
transportation, in-home services
and inpatient care (when needed).
In Bernalillo County, this service
is available through InnovAge. To
qualify for PACE, the senior must
be aged 55+ and live within the
county or within 30 minutes travel
from center. Free for recipients
of both Medicare and Medicaid.
Private pay also accepted.
POA: Power of Attorney – a legal
form designating someone else to
act on your behalf if you become
unable to chose medical care for
yourself.
