RETURN TISSUE INSTRUCTIONS

Eversight is committed to honoring the gift of sight by ensuring every tissue is placed for transplant or eye and vision research . If you are unable to use the tissue you received , please return it same day of scheduled surgery to Eversight following the steps below so the tissue can be placed with another patient .

1 . Contact Eversight Tissue Placement immediately at ( 866 ) 900-8119 . Please have your fax number or email ready to receive a Returned Tissue Documentation form .

Due to contamination risk , Eversight cannot accept returned tissue if tamper evident seal on the tissue container has been removed . If the seal is broken the surgery center will be billed .

2 . Complete and return the form to the fax or email provided . A package pick-up will be arranged to return the tissue to Eversight .

3 . Prior to repackaging , store corneal tissue in a monitored refrigerator or on wet ice . DO NOT FREEZE .

4 . To prepare the tissue for return shipment , reuse the Eversight packaging to keep tissue properly cooled . The styrofoam coolers are validated to maintain a cooled temperature for 24 hours . The eye bank must receive returned tissue within this timeframe .

5 . Place the tissue container into a small , sealable plastic bag to protect the label .

6 . Place the bagged tissue container into the dry foam insert from the original packaging , and place it inside a larger zip-lock bag . Expel air from the bag and seal it . Place the bag inside the styrofoam cooler .

7 . Fill a large , no-leak bag with ice so that the remaining cooler space is filled . Large tissue coolers may require more ice . Expel air from the bag and seal it .

8 . Place the lid on the cooler and secure it with tape .

9 . Place the styrofoam cooler into a plastic liner bag ( if available ), and place this inside the cardboard box . Seal the box with packing tape .

10 . If directed , attach a shipping form to the shipping box lid .
