n election will be held on Tuesday, October 14, 2014 for the purpose of electing Council representatives to the
College of Physicians and Surgeons of Ontario for Districts 5 and 10.
Members interested in becoming candidates must submit their nomination and conflict of interest forms to the
Registrar no later than 4:00 p.m. Tuesday, August 26, 2014.
The nomination form must bear the signature and registration numbers of at least five members entitled to vote in
the electoral district for which the candidate is nominated.* The candidate nomination and conflict of interest forms
are included in this issue. Additional forms can be obtained by calling Tanya Terzis at (416) 967-2600 ext. 362 or
1-800-268-7096 ext. 362, or by downloading them from the College website at www.cpso.on.ca.
On-line voting will be open from Thursday, September 18 to 4:00 p.m. Tuesday, October 14. Any votes/ballots
received later than 4:00 p.m. on October 14, 2014, will be considered ineligible. Further information about the voting process can be found at www.cpso.on.ca. Eligible voters will also receive additional information about the voting
process in the election mailing.
Candidates will be notified of the results on Wednesday, October 15, 2014.

*It is recommended that seven nominators be provided in the event that a nominator is not in good standing. Each nominator
