Mobile App Spotlight

The ACC Statin Intolerance App guides clinicians through the process of managing and treating patients who report muscle symptoms while on statin therapy . Clinicians can use the app to evaluate possible intolerance to a patient ’ s current statin prescription , follow steps to manage and treat a patient who reports muscle symptoms on a statin , and compare statin characteristics and drug interactions to inform management of LDL-related risk . The trials include : The Heart Outcomes Prevention Evaluation ( HOPE )– 3 trial : Blood-Pressure and Cholesterol Lowering in Persons without Cardiovascular Disease ( New England Journal of Medicine ) and Effect of Early Administration of Beta Blockers in patients with STEMI before primary PCI : The EARLY-BAMI trial ( JACC ). Certified Patient Case
A 73-year-old woman presents with a history significant for hypertension , diabetes mellitus , paroxysmal atrial fibrillation for 4.5 years on diltiazem and dronedarone , previously anticoagulated with warfarin and rivaroxaban within the last 2 years . Seven months ago , she developed headache , ataxia , confusion , and visual disturbance and was found to have a spontaneous left intracerebellar hemorrhage with extension into ventricles resulting in hydrocephalus , along with multiple old punctate areas of hemorrhages in bilateral cerebral hemispheres . She was treated with external ventricular drainage and discontinuation of anticoagulation . She recovered well from the event except for the ataxia . What would you do ?
