YEAR TWO 1. Worked with the school district to implement health education into the high school curriculum. The HOPE course (Health Opportunities through Physical Education) was implemented in all high schools. This course is a graduation requirement for all students graduating in 2017 and beyond. 2. Year two also saw the district-wide implementation of Fitnessgram; the results of the assessment provided two parent reports that were sent home. 3. This strategy also researched and selected the 5210 campaign for use in aligning physical fitness and nutrition messaging among partners. The campaign was set to launch in 2015. YEAR THREE 4. The 5210 Campaign was promoted heavily throughout the district by a multitude of agencies including WIC, local pediatricians, and the school district. The team implemented a timeline which included a presentation at the annual Building a Healthier Polk 2015 Kick-off, the Polk County School Board School Health Advisory Council, and the Polk County School Board Head Start Parent Advisory Committee. See Addendum A for talking points. 5. The Polk County School Board created a proclamation to support 5-2-1-0 education throughout schools. Strategy members assisted with the implementation of 8 messaging in cafeterias, education in health and physical education curriculum, and outreach to parents. 6. Drafted a 5-2-1-0 toolkit as a resource to support teachers, after-school care providers, and community educators. 7. Organized and implemented the 5-2-1-0 Healthy Habits Poster contest for schoolbased and community summer camp participants. The posters were created by school-aged children throughout the district to disseminate the 5210 message. 8. Partnered with Legoland Florida to host the 5-2-1-0 Heathy Habits walk to recognize poster contest winners.
