YEAR THREE
In year three , Building a Healthier Polk launched its Healthy Habits Campaign promoting the 5-2-1-0 Initiative . This initiative encourages healthy eating and active living to prevent and reduce obesity through encouraging a life style with simple and healthier options by choosing :
• 5 – fruits and vegetables every day
• 2 – hours or less of screen time per day
• 1 – hour or more of physical activity every day
• 0 – sugar-sweetened drinks
The Primary Care Physicians Strategy workgroup partnered with local pediatric practices who committed to participate in a 6-month project . These practices were provided with 5-2-1-0 prescription pads to be used to facilitate conversations with patients and families regarding healthy lifestyle choices .
METHOD
Four clinical practices were contacted and agreed to participate in a 5-2-1-0 prescription project for 6-months . The purpose of this project was to provide a simple prescription for providers to use to facilitate a conversation with children and their families regarding healthy lifestyle choices .
Pediatricians were provided with prescriptions designed to describe the 5-2-1-0 Initiative healthy habit goals . These prescriptions included a place for the child to designate which healthy habit ( s ) they would commit to work on and a place for the physician to document the age and BMI percentile of the child .
Prescriptions for the Nature Play program were also given to pediatricians . This program provides free 1-day admission to a Florida State park with a goal of increasing physical activity through utilization of state parks . It is the result of a partnership between the Florida Department of Environmental Protection , Florida Department of Health , and Get Outdoors Florida .
