Bay of Plenty Community Care Coordination (CCC) was launched as a demonstration site in March this year with referral management and triage for District Nursing Services and the Falls Prevention - In Homes Strength and Balance Programme. The team includes registered nurses Lucia Da Silveira, Rozanne Young, and Teresa Wynyard, and administrators Pauline Louie and Rowena Strugnell. • • • The BOPDHB has contracted Body in Motion Physio & Rehab to develop and provide a Falls Prevention programme. This will see Physiotherapists and physiotherapy supervised nurses visit patients at home and follow them up via phone calls over a 12 month period. For nursing referrals, we aim to ensure that care is directed to the most appropriate community nurse provider and the referral information is relevant and complete. We work closely with district nurses, in patient staff and primary healthcare to achieve this. For example, patients who require routine wound care and are able to attend a clinic are funded for package of care at most general practices. CCC assesses this and refers suitable patients to primary healthcare where appropriate. CCC asks that referrers please be mindful of the following when referring to district nurses: • • Consider if the patient really needs a home visit by a district nurse or if they can they attend a clinic and note this on the referral. Remember the patient may not get a visit by a district nurse because they may be referred to another nursing service, so it is best to inform them that they are referred to a ‘nurse’, rather than a district nurse. Short- term services (STS) can take up to five working days before the service starts. Therefore early referrals ensure • timely care and this is especially important if the person is being discharged towards the end of the week. All referrals for ACC STS must be sent directly to one of the providers on the ACC 705, not district nurses or CCC. It is challenging to provide care when referrals are received late. If a patient needs to be seen on the same day; or if you need to refer for a visit for the next day and it is after 3pm please contact district nurses to discuss the referral or consider changing the date and referring as per normal process. Please send accurately competed additional paperwork that’s required for the care to be effectively completed. Examples of these include ‘Outpatient Medication Chart’ (print room order number 8285); wound care plans, drain charts etc. Remember to record on the medication chart the date and time of when the last dose was administered. If CCC does not receive accurate paperwork, the requested care cannot be delivered. We encourage all departments that are not using WebPAS to start doing so as it is a lot more efficient and safer.
