MAKING WORKING SPACE Imagine the working space ! You have already the maps in your mind. Just pick it out! Ground Plans The first step in BESS is to make working space. The one of the hardest procedure for the recruit of the BESS is to “bring a facet joint in front of your eyes”. Everyone who tried BESS for the first time spent one or two hours to secure working space. You can touch and feel the top of the facet with a certain instrument like a freer. But it is hard to see and differentiate the facet from the surrounding back muscle. When the saline was infused into the deep space, the back muscle was going to be swollen and the facet came to be covered down with heavy muscle. Whether or not you can get the working space in your direct visual field around the facet is important to proceed with the procedure. 8 InU’rMind 1. Don’t be worry! Everybody failed at first. 2. If you rush down without working space, heavily swollen muscle and bleedings from all sides try to defeat you down. 3. The first Base Camp. a. b. c. Detachment of back muscle at the basement. Securing of working space in the mid-line for crossing-over to contralateral approach.
