RELEASE OF LIABILITY
I hereby covenant not to sue and agree to indemnify, and save, and hold harmless “P&M Trail Riders, Inc.”, City of Mesa, its agents, employees and representatives and all other
in any way connected with the products and operation of the event and the facilities from any claim, demand or liability for personal injuries or property damage arising from
participation or use and enjoyment of the event and facilities. (In accordance with ARS Section 12-553 Limited Liability of Equine Owners and Owners of Equine Facilities-HB3467).
The consent will remain in effect for a period of one (1) year. THE UNDERSIGNED HAVE READ AND VOLUNTARILY SIGN THE COVENANT AND INDEMNITY AGREEMENT and further
agree that no oral representation, statement nor inducements apart from the foregoing written agreements have been made.
I acknowledge that I may be photographed while on site and/or participating in P&M activities. I agree to allow my photo, video, or film likeness to be used for any legitimate
purpose by the event holders, producers, sponsors, organizers, and assigns.
