OUR MISSION is to provide high-quality, comprehensive medical and
dental care, patient advocacy and related services to people who need them
most, regardless of their ability to pay.
WE ARE a nonprofit, Federally Qualified Health Center (FQHC)
dedicated to providing health care to those experiencing homelessness,
individuals living in public housing and the underserved in Cleveland,
Ohio.
WE PROVIDE primary medical care, comprehensive dental care,
patient advocacy and supportive services to our patients and clients. This
includes:
• Annual physicals, well child exams and women’s health
ser vices;
• HIV and STD testing, counseling and treatment;
• Chronic disease management, podiatry, physical therapy and
referrals to specialty care;
• Electronic Health Records and E-prescribing;
• Medical case management, short-term behavioral health
counseling and eligibility/prescription assistance;
• Dental exams, extractions and restorative care including
dentures and partials;
• Outreach to individuals experiencing homelessness.
WE MAKE health care affordable. Our patients pay on a sliding fee
scale based on income. In addition to caring for the uninsured, we accept
many types of insurance, including Medicaid and Medicare.

In 2013, Care Alliance served over 10,000 patients through more than
34,000 visits. More than half of our patients are experiencing homelessness.

TRANSFORMATIONAL INITIATIVE
Care Alliance hired John Snow, Inc. to develop a detailed strategic plan accounting for our significant
expansion, strategic initiatives and key partnerships. The following recommendations were
proposed to ensure we continue to work towards improving the health status of the communities
we serve: 1) Stay True to our Homeless Mission, 2) Meet Unmet Need, and 3) Maximize Capacity.
