Endorsement 2013 Endorsement Procedures office staff may request any documents reasonably believed to be necessary in the determination of a player’s eligibility. The player has the burden of proof. Proof of Domicile A player’s domicile shall be the domicile of that player’s “parents”. Domicile is hereby defined as the bonafide, true and permanent home of the player’s “parents”. In the event that the player’s parents are divorced or legally separated, the custodial parent or the legal custodian who in fact resides with the player, shall be presumed to be the player’s parent for determining domicile. In the event of a dispute as to the eligibility of a player under this section, the following factors shall be used to determine domicile: 1. A photocopy of the “parents’” most recent state income tax return or a printed copy of the electronic file form. 2. A photocopy of the “parents” most recent federal income tax return or a printed copy of the electronic file form. 3. A copy of a Declaration of Domicile, signed by the “parents” proving that the player’s “parents” live in the USTA/Midwest Section. The document must be notarized. 4. Affidavit of domicile executed by the head of the household. 5. Photocopies of “parents” and players (when applicable) driver’s licenses and automobile registration (s). 6. Notarized copy of “parents” voter registration. 7. Photocopy of Homestead Tax Credit (all states except Ohio, unless the “parents” meet the special requirements in Ohio) The above list is not intended to be definitive or all inclusive and other factors may be considered by the USTA/Midwest Section Junior Competition Committee, or may be submitted by the player’s parent, to assist in determining the players’ domicile. The USTA/Midwest Section office staff may request any documents reasonably believed to be necessary in the determination of a player’s eligibility. The player has the burden of proof. A District, player or parent may submit a request for reconsideration of a domicile ruling to the USTA/Midwest Section Junior Competition Committee. The decision of the USTA/Midwest Section Junior Competition Committee or its appointed subcommittee on the reconsideration request may be appealed to the USTA/Midwest Section Board of Directors. The decision of the USTA/Midwest Section Board of Directors shall be final on the issue of domicile. All requests for reconsideration must be filed with the USTA/Midwest Section Office at least 30 days, prior to the entry deadline of any closed tournament in which the player wishes to enter. Any additional documents requested by the USTA/Midwest Section Junior Competition Committee shall be submitted within five (5) business days after their request. The “parents” may submit any additional documents at anytime prior to the reconsideration review by the USTA/Midwest Section Junior Competition Committee or its appointed subcommittee. The reconsideration shall be considered based upon all documents submitted to the USTA/Midwest Section Junior Competition Committee. Any falsification of information will result in a minimum one-year revocation of junior tennis privileges in the USTA/Midwest Section. A player suspended for falsification of information may apply for re-instatement. The player may apply 30 days prior to the effective suspension one year anniversary date. Failure to apply for reinstatement will cause the suspension to continue until such time as re-application/re-instatement request is approved for said player by the USTA/ Midwest Section Junior Competition Committee or its appointed subcommittee. Endorsement of USTA/Midwest Section’s Quota and Alternates for USTA National Junior Events The following criteria apply for all USTA National Junior events: • A player may request endorsement in only one age division. Minimum Requirements for Inclusion on the USTA National Clay Court Championships Endorsement List All players that appear on the USTA/Midwest Section Standing List at the conclusion of all District Qualifiers and prior to the USTA/Midwest Section District Team Cup BG12,14, 18 and the USTA/Midwest Section Marian Wood Baird Cup, will be endorsed to the event. Please see “Endorsement Grid” for additional information. Minimum Requirements for Inclusion on the USTA National Championships Endorsement List All players that appear on the USTA/Midwest Section Standing List, which will be generated after the completion of the USTA/Midwest Section Closed Junior Outdoor Championships, will be endorsed to the event. The winners of each of the three Designated tournaments taking place in the preceding January, February and March shall be automatically included in the Sectional quota. In addition, the top six finishers at the USTA/Midwest Section Closed Junior Outdoor Championships (main draw semifinalists and FIC finalists) shall automatically be included in the Sectional quota. Any remaining spots in the Sectional quota and all alternate spots shall be filled in the order of the endorsement list, which will be generated after the conclusion of the USTA/ Midwest Section Closed Junior Outdoor Championships. USTA Zone Team Championships for 16-, 14-, and 12-and-under Players The USTA Zone Team Championships are non-elimination team events providing intersectional competition for players selected to the Zonal teams. The USTA/Midwest Section will endorse players in the 12, 14 and 16 age divisions. Players may be endorsed to ANY or ALL USTA Zone Team Championships if they meet the endorsement requirements. The emphasis of the team events will be on a combination of competition and development. Limited coaching by designated coaches during play will be permitted. Team matches will include singles, doubles and mixed doubl
